1. Open to current NUS staff only.
2. Your appointment time has been reserved exclusively for you. Please note that missed appointments or cancellations will be subjected to a full-service charge of $138.00 (subject to prevailing GST). Exceptions may be made for extenuating circumstances such as illnesses. A valid medical certificate (MC) must be submitted to the University Health Centre (email: Miss Lai Xiuli / Miss Clarissa Fong) within 48 hours of your appointment, at our discretion.
3. Online pre-registration is required. Walk-in appointments will not be entertained.
4. By submitting the registration form, you consent to the collection, use, and disclosure of your personal data for the sole purpose of scheduling, processing, administration, and management of the event, in accordance with the Personal Data Protection Act (PDPA).
5. Do not have breast symptoms such as breast lumps or nipple discharge.
6. Women aged 40 years old & above (Singapore citizen, Singapore PR & Non-Resident^).
7. Not pregnant and have not been breastfeeding for the past 6 months.
8. Have not gone for mammography in the last: 12 months (40-49 years old)^^, 24 months (50 years old & above)
9. Please wear a mask If you are unwell or exhibiting symptoms such as cough and sneezing.
10. Patients with mobility aids are advised to have their mammogram screening done at a breast screening centre.
11. Please take note that all healthcare institutions need to carry out proper documentation in relation to the provisionof your care. As such, accurate verification of a patient’s identity to ensure that medical care and treatment is provided to the right patient. The requirement to maintain accurate, complete and up-to-date medical records for the purposes above is supported by Regulations 12(1) and 12(1)(1A)(a) of the Private Hospitals and Medical Clinics Regulations. Should you require any clarification, please do not hesitate to write to NHGD.
