NUS-IRB Submission Forms

These forms are accessible to NUS staff only. For non-NUS investigators, please contact us.

01.      NUS-IRB Application Form

           NUS-IRB Application Form (Social Sciences)

-          All sections should be completed.

02.      Exemption from Full NUS-IRB Review Form

-          Please read our guidelines on ethical review of research using human cell lines for more information.

03.      E-Declaration on the use of commercial cell lines (Non-Embryonic)

-          Please read our guidelines on research involving commercial cell lines from human source (Non-Embryonic) only.

04.      Human Sample Types Template

-          All human biological sample types used for a research are to be included in this table.

05.      Protocol Amendment Form

(Use this form to request approval to changes to an approved research)

 

06.      Continuing Review Report Form

(Annual Progress Report)

-          NUS-IRB review is an ongoing process. Regular re-evaluation ensures that research is conducted responsibly. By doing so, we hope to understand better the needs and problems of researchers.

07.      Final Report Form

(Completion and Closure of NUS-IRB approved research)

-          A final report/summary of the research should be submitted to NUS-IRB within 3 months after completion of the study.

08.      Local Serious Adverse Event Report Form

-          NUS-IRB should be promptly informed of all adverse events that are both serious and unexpected.
