A. New protocol application

All new protocol applications are now submitted through the Integrated Online Research Compliance System (iORC) for IACUC review. Forms available in the iORC are:

1. Use of animals for research

This form is for application to use animals for research experiment and testing.

Separate applicable Procedure A-F forms (see 8 below) are required to provide details of procedures.             

2. Use of animals for biodiversity/field study

This form is for application to use animals for field study, such as ecological assessment and field collection.

If the field study involves specific species and number of animals which are subject to experiments, the form for ‘Use of Animals for Research’ should be submitted instead.     

3. Use of animals for classroom teaching

This form is for application to use animals for practical classes, workshop and training course.

Separate applicable Procedure A-F forms (see 8 below) are required to provide details of procedures.

4. Use of animals for undergraduate/school student project 

This form is for application to use animals for undergraduate student project, such as Honours project, Undegraduate Research Opportunities Programme in Science (UROPS), Science Resaerch Programme (SRP) and Science Mentorship Programme (SMP).

Separate applicable Procedure A-F forms (see 8 below) are required to provide details of procedures.

5. Use of animals for other scientific purposes 

This form is for application to use animals for other scientific purposes such as imaging (photography and video recording), exhibition and demonstration of research animals.

6. Maintaining a breeding colony

This form is for application to maintain a breeding colony. Animals bred under this breeding protocol are for transfer to a research protocol for use.

7. Use of animal tissue

This form is for application to use animal tissues obtained through commercial sources, tissue sharing from another approved protocol, and any other sources where animals are not specifically euthanized for harvesting the tissue required.

If animals are specifically euthanized to obtain the tissue required, The full protocol form (i.e, the applicable form 1-4 above) should be submitted instead.  

8. Procedure Forms

The following procedure forms are to be submitted along with applicable protocol forms 1-7 above, for providing details of procedures under the protocol.

Procedure A: Surgical Procedure

Procedure B: Non-surgical Procedure

Procedure C: Genetically Modified Animals

Procedure E: In-vivo Maintenance of Hybridomas / Ascites 

Procedure F: Housing Outside Licensed Area (>12 hr)

 

B. Amendment of an approved protocol

 

1.  Amendments of approved protocols processed through iORC.

Amendments of approved protocols processed through iORC are submitted through the iORC for IACUC review.

2. Amendments of protocols not processed through iORC

Amendment of approved protocols not processed through IORC (i.e., applications submitted before the launch of iORC on 20 Nov 2012 and approved after review) are submitted using the following forms for IACUC review.

Completed main Amendment form and the applicable Appendixes are submitted (in soft copy) to IACUC Office  dprnmya@nus.edu.sg

 

Application to amend an approved protocol

This form is for Application to amend an IACUC approved protocol.

Submit the main Amendment form and the Appendices (1-5), as listed below, that are applicable to your application.

C. Annual Protocol Review

Annual Protocol Review

All animal use protocols are to be reviewed by IACUC annually. The Annual Protocol Review (APR) forms listed below should be completed for IACUC review every year. They also serve as final reports upon completion of the projects / workshops.

Annual Review for Research Projects This form is for annual review of protocols for research projects including student projects and biodiversity / field studies. It is to be reviewed annually on the anniversary of the protocol approval date. Completed forms should be submitted 1 month before the due date.

Annual Review for Classroom Teaching / Workshops This form is for annual protocol review of protocols for classroom teaching / workshops. It is to be reviewed annually on the anniversary of the protocol approval date. Completed forms should be submitted 1 month before the due date.

Annual Review for Maintaining a Breeding Colony This form is for the annual review for the protocols for a breeding colony. It is due annually in January and should be submitted by the 5th, irrespective of the approval date.

D. Reporting animal care and use concerns

Reporting animal care and use concerns

To help ensure that laboratory animals receive humane care and use or treatment in accordance with the highest ethical standards, laws, regulations and policies governing animal research, the IACUC will review and, if warranted, address any animal related concerns raised by the public or Institutional employees. The Reporting Form and the SOP are established to ensure that concerns are communicated and addressed in a timely and systematic manner and, when necessary, appropriate corrective actions are taken promptly.

The confidentiality of those who report concerns, as well as anyone against whom allegations are directed while allegations are under investigation, are protected. The complainants are also protected from reprisals under the NUS policy on research integrity and research misconduct.
