Please bring the following items:

Medical Examination Form
Please fill in the NUS Admission Medical Examination Form before coming for medical screening at UHC.
2.
Corrective Lenses
If you wear spectacles or contact lenses, please bring them on the day of your medical examination.
3.
Medical Reports
For those with pre-existing medical conditions, please bring the relevant medical reports and supporting documents.
