UHC, Health Service (Clinic) has changed its addres to
PREADMISSION MEDICAL EXAMINATION 2) All Other incoming Graduates (AY2010/2011) are to book their appointment from 21 June - 06 August 2010 (Open for registration from 14 Jun) 3) No booking is necessary for all other intakes and offers that fall outside the period stated above. Singaporeans and Permanent Residents are strongly encouraged to book their medical examination dates early. We expect a congestion later in the booking period when our fellow international students arrive in Singapore for medical examinations. NO FASTING IS REQUIRED FOR THE MEDICAL EXAMINATION. Graduate Students Please Take Note: You must have accepted the offer prior to make the booking. In case you see message ‘You are not allowed to register for this event.’. Please check your offer status in your application PLEASE BRING THE FOLLOWING ITEMS (i) Medical Examination Form If you are an international student, likewise detach and fill in the Student Pass Form in the Freshmen Guide in addition to the Admission Medical Examination Form before coming for medical screening. (ii) Corrective Lenses (iii) Medical Reports PLEASE DRESS APPROPRIATELY You will be undergoing a full physical examination and a chest X-ray at UHC, so please come dressed simply and appropriately. To ensure that objects that obstruct proper reading of the X-ray films are removed when the chest X-ray is taken, no necklace or nipple rings are allowed during the X-ray examination. For this same reason, we advise ladies to wear a PLAIN blouse or T-shirt on the day of examination. In particular, the shirt or blouse should not have any of the following:
You will need to purchase a disposable x-ray gown at a nominal fee if your attire is found to be unsuitable for the X-ray examination.

| 1. Payment Please present the completed forms at the payment counter. Please make payment at the payment counter, and keep the receipt. ![]() The Medical Examination Fees are as follows (Prices are subjected to change without prior notice) :

| 2. Parameters Your height, weight, eyesight and colour vision will be checked. ![]() |

| 3. Blood Test It is not required for everyone to go for a blood test for the medical examination. Blood tests are needed only for (a) Student Pass Medical Examinations, (b) Hepatitis B Screening (c) Chickenpox Screening. If you require a blood test, please fill up Name/NRIC/PP/FIN on Blood Test form and tubes. After doing so, proceed to treatment room and leave the form in the tray at the door. ![]() |

| 4. Blood Pressure Your blood pressure will be measured. ![]() |

| 5. Urine Test It is required for everyone to do a urine test. Please inform the staff if you have had your menses within the past 5 days. Please drink plain water (no sugar) before coming for your medical examination. ![]() |

| 6. Medical Review by the Physician The doctor will review your history and conduct a medical examination. ![]() |

COLLECTION OF MEDICAL REPORTS For International students, the Student Pass Medical Report, chest x-ray and blood test results must be collected personally. |

Exemption Graduate students (excluding those enrolled in medicine, nursing and dentistry and courses that require additional health requirements) who are NUS staff members or are being employed in local companies may be exempted from the pre-admission medical examination if they have done a SIMILAR medical check-up not more than six months before the time of registration. University Health Centre shall review the exemption on a case by case basis and reserve the right to refuse an exemption. Note |
