ONLINE BOOKING FOR YOUR MEDICAL EXAMINATION

1) Returning National Servicemen are to book their medical examination appointment from 1 - 30 April 2009

2) All Other incoming Undergraduates (AY2009/2010) are to book their appointment from 22 June - 07 August 2009

Singaporeans and Permanent Residents are strongly encouraged to book their medical examination dates early. We expect a congestion later in the booking period when our fellow international students arrive in Singapore for medical examinations.

Please choose either the morning (8:30-11:30am) or afternoon (1:30-4:30pm) slots for the medical examination. (We are closed on Saturdays, Sunday and Public Holidays)

For female candidates, please book a date for medical examination at least 5 days after the end of your menses. This is because menstruation will affect the results of the urine test.

NO FASTING IS REQUIRED FOR THE MEDICAL EXAMINATION.

You will need your application number and PIN number from the Office of Admissions to submit the online booking. If you have forgotten or misplaced your application number or PIN number, you can retrieve them via the Office of Admissions Online Application Status Facility at https://aces01.nus.edu.sg/uasonline/Login

PLEASE BRING THE FOLLOWING ITEMS

(i)         Medical Examination Form
             Please fill in the Admission Medical Examination Form (found in the Freshmen Guide/Offer package) before coming for              medical screening at UHC.

             If you are an international student, likewise detach and fill in the Student Pass Form in the Freshmen Guide in addition to              the Admission Medical Examination Form before coming for medical screening.

(ii)         Form M
             To do the medical examination at UHC, please bring Form M found in the offer pack that has been sent to you. If the                           medical examination is to be done at another medical centre, please attach Form M to the completed medical report, and              submit both to UHC.

(iii)        Corrective Lenses
             If you wear spectacles or contact lenses, please bring them on the day of your medical examination.

(iv)        Medical Reports
             For those with pre-existing medical conditions, please bring the relevant medical reports and supporting documents. 

PLEASE DRESS APPROPRIATELY

You will be undergoing a full physical examination and a chest X-ray at UHC, so please come dressed simply and appropriately.

To ensure that objects that obstruct proper reading of the X-ray films are removed when the chest X-ray is taken, no necklace or nipple rings are allowed during the X-ray examination.

For this same reason, we advise ladies to wear a PLAIN blouse or T-shirt on the day of examination. In particular, the shirt or blouse should not have any of the following:

  • buttons
  • logos or designs that are printed or embroidered
  • pockets
  • any collar on the shirt/blouse

You will need to purchase a disposable x-ray gown at a nominal fee if your attire is found to be unsuitable for the X-ray examination.

 

MEDICAL EXAMINATION PROCESS

The medical examination process involves the following:

1. Payment
Please present the completed forms with Form M at the payment counter.
Please make payment at the payment counter, and keep the receipt.
Then collect and fill up the Medical Card (if any).



The Undergraduate Medical Examination Fees (incl. of GST) are as follows:

Standard Medical Examination (includes Chest X-Ray) $30
Medical Examination without Chest X-Ray $25
Standard Medical Examination with Student Pass Examination $45
Standard Medical Examination with Hepatitis B Screening $48
Standard Medical Examination with Hepatitis B & Chickenpox Screening $90
Standard Medical Examination with Student Pass Examination, Hepatitis B & Chickenpox Screening $105
2. Parameters
Your height, weight, eyesight and colour vision will be checked.
3. Blood Test
 It is not required for everyone to go for a blood test for the medical examination.
Blood tests are needed only for (a) Student Pass Medical Examinations, (b) Hepatitis B Screening (c) Chickenpox Screening.
If you require a blood test, please fill up Name/NRIC/PP/FIN on Blood Test form and tubes.
After doing so, proceed to treatment room and leave the form in the tray at the door.
4. Urine Test
It is required for everyone to do a urine test.
Please inform the staff if you have had your menses within the past 5 days.
Please drink plain water (no sugar) before coming for your medical examination.
5. Blood Pressure
Your blood pressure will be measured.
6. Medical Review by the Physician
The doctor will review your history and conduct a medical examination.
7. Chest X-Ray
Bring the X-ray Form with you to the X-ray room

COLLECTION OF MEDICAL REPORTS

The NUS Medical Examination Report need not be collected. We will send the medical examination result to your School or Faculty.

For International students, the Student Pass Medical Report, chest x-ray and blood test results must be collected personally.
The reports are available for collection from UHC one week after testing. Please bring along your passport when collecting your reports.

You are required to prepare an authorization letter if you are sending someone to collect your reports on your behalf.
