The purpose of this feedback questionnaire is to gather information on your learning experience, as well as your responses to the course and the teacher(s). Please think of these questions as eliciting your subjective perceptions on various aspects of the course and the teacher(s) involved in the course. The information you provide will be useful to your teacher(s) as well as to the University in the ongoing efforts to enhance the quality of education at NUS. The data you provide will be treated as confidential. The evaluation results will be computed using the total scores from the responses from all students and will be released to the affected teachers AFTER your results have been finalised.

The feedback consists of two parts: Part I & Part II covers your responses to the course as a whole, while Part III covers your responses to the teachers (lecturers, seminar facilitators, tutors, and instructors) of the course. Please answer all quantitative questions.

Notice:

1. For All Faculty, the system is open for access daily from

   25 Jul 2008 (0800 Hrs) to 31 Jul 2008 (2359 Hrs)

   If possible, please avoid last minute submission.
   


2. Undergraduate students in the School of Medicine [SOM] provide feedback on their medicine course through the relevant SOM Feedback Exercise.
   
   However, SOM undergraduate students who have taken modules from other Faculties during Semester 4 AY2007/2008 should complete this online students feedback, please click on YONG LOO LIN SCH OF MEDICINE link on the left to access the system.
   
   
3. Students on Industrial Attachment or did not need to attend any classes/exams in NUS for this semester; did not need to take part in this online students feedback.
   
   

For Graduate/UnderGraduate Students, please click on your Faculty’s URL link else OTHER FACULTY link on the left to access the system.

For Non-Graduating/Exchange Students, please click on NON-GRADUATING link on the left to access the system.

Note:
If you encounter the following error:
ERROR 700 -- The proxy-cache server is not configured properly for
SSL/SNEWS or other tunnelling requests.

Please perform the following steps:
For NUS Dialup, you need to set the proxy:
In IE -> Tools -> Internet Options -> Connections -> LAN Settings
Click Use Automatic Configuration Script
In the address field, key in : http://www.nus.edu.sg/proxy/config.pac
In Netscape -> Edit -> Preferences -> Advanced -> Proxies
Click Automatic Proxy Configuration
In the Configuration location (URL) field, key in : http://www.nus.edu.sg/proxy/config.pac

For Other ISPs:
In IE -> Tools -> Internet Options -> Connections -> LAN Settings
Unclick all the settings (in other words disable the proxies)
In Netscape -> Edit -> Preferences -> Advanced -> Proxies
Click Direct connection to the Internet
