Student Feedback System for Graduate/Undergraduate/Non-graduating Students
The purpose of this feedback questionnaire is to gather information on your learning experience, as well as your responses to the course and the teacher(s). Please think of these questions as eliciting your subjective perceptions on various aspects of the course and the teacher(s) involved in the course. The information you provide will be useful to your teacher(s) as well as to the University in the ongoing efforts to enhance the quality of education at NUS.
The data you provide will be treated as confidential. Please be assured that your feedback is strictly anonymous—the identity of all students who provide feedback is protected. The evaluation results will be computed using the total scores from the responses from all students and will be released to the affected teachers AFTER your results have been finalised.
The feedback consists of two parts: Part I & Part II covers your responses to the course as a whole, while Part III covers your responses to the teachers (lecturers, seminar facilitators, tutors, and instructors) of the course. Please answer all quantitative questions.

Undergraduate students in the School of Medicine [SOM] provide feedback on their medicine course through the relevant SOM Feedback Exercise.
However, SOM undergraduate students who have taken modules from other Faculties during Semester 2 AY2012/2013 should complete this online students feedback.
Students on Industrial Attachment / Exchange or did not need to attend any classes/exams in NUS for this semester; did not need to take part in this online students feedback.
