| (A)Things to do before reporting for registration |

Before reporting in person for the actual registration, all new students are required to do the following: |

| |
1. |
Online Tracking of Admission Related Forms (for undergraduate students only) |
| |
|
As part of the admission conditions, incoming students are required to submit the following forms to the Registrar’s Office by the deadline as stipulated in the offer letter:
- Form P (previously known as Form A): Photograph for Student Card
- Form 1: Release of Liability
- Form 2: Authorisation of Medical Procedures (for International Students only)
- Form 3: Appointment of Local Representative (for International Students with a local representative in Singapore)

2. |
Online Pre-Registration |
| |
|
All students will have to complete the Pre-Registration process online before reporting to the registration centre. At the Pre-Registration System, you can update some of your personal particulars, obtain your student number with the student account information and download forms which you need to submit at the registration centre. To access the Pre-Registration System, you have to use the Admission Application Number and PIN issued to you earlier (for graduate research students who did not apply for admission online, they will be given a PIN to access the Pre-Registration System).For those who have forgotten the Admission Application Number and/or PIN, please go to Undergraduate Admission System if you are an undergraduate student or Graduate Admission System if you are a graduate research student. |

3. |
Pre-enrolment Medical Examination |
| |
|
All undergraduate and graduate students are required to undergo a pre-enrolment medical examination prior to entering NUS. The cost of the medical examination shall be borne by the student. The medical examination will consist of the following:
Physical Examination
Laboratory Examination*
Chest X-ray
HIV Test (for international students only)
* Note: Ladies are advised to go for the medical checkup after their menstruation as a urine test is required.
This medical examination can be done by your own physician or at the University’s Health, Wellness and Counselling Centre (UHWC), Level 4, Yusof Ishak House (YIH), NUS. For your convenience, you are strongly encouraged to have your medical examination done at UHWC. The cost of the medical examination and chest X-ray is S$30 (excluding GST) for Singaporeans while the cost of the medical examination without the chest X-ray is S$25 (excluding GST). For international students, the cost is $45 (excluding GST) which also covers the HIV test for student pass application. Please bring along a blank copy of the NUS medical examination report form on the day of your medical examination (for graduate students, the medical form is enclosed with the enrolment booklet that will be sent to you). If you choose to consult your own physician, the form must be completed by you and your physician and submitted, with the chest x-ray film and reports of the laboratory tests attached, on the day of registration. International students who opt to have their medical examination done at UHWC can collect the completed medical report form for the student pass application 5 working days after the checkup.
The University may require you to undergo further medical examinations, such as a blood test, before your admission is confirmed. Nonetheless, unless you hear from us to the contrary, you may assume that the University has received your medical report and that it is in order. |

(ii) |
Special consideration for students admitted to Medicine,
Dentistry, and Nursing |
| |
|
|
Compulsory screening for Hepatitis B virus antigen and antibody for all medical, dental and nursing students must be done less than 3 months from the registration date. Please note the following: |
| |
|
|
- Those who have completed screening and immunization can submit their certified results at UHWC.
- Those who have been screened more than 3 months from the registration date and are not immunized will have to repeat the screening test. Immunization should commence not more than 1 month from the screening date.
- Screening and immunization can be done with any registered general practitioner or at UHWC.
- The cost of screening at UHWC is S$15 (inclusive of 5% GST). The cost for immunization (total of 3 injections over a period of 6 months) is S$15 per injection (inclusive of 5% GST)

(iii) |
Special requirements for international students to obtain Student Pass |
| |
|
|
It is compulsory for international students to undergo a HIV test as part of the medical examination required for their Student Pass. For this medical examination, please use the form prescribed by the Singapore Immigration & Checkpoints Authority (ICA). You may wish to particularly take note of the following: |
| |
|
|
- The HIV test and medical examination can be done in the country of origin or in Singapore. (Note : The medical report must be completed in English.)
- Laboratory test reports and chest x-ray films, together with the Medical Report for Student’s Pass, must be submitted to the Immigration & Checkpoints Authority (ICA) within 3 months of issue.
- The medical examination can be done at UHWC. The cost of the medical examination, including chest X-ray and HIV test, is S$45 (inclusive of GST)

Other things to note |
| |
|
|
In order to facilitate the medical examination, please take note of the following: |
| |
|
|
- If you wear spectacles or contact lenses, please bring these along on the day of your medical examination.
- For those with pre-existing medical conditions, please bring the relevant medical reports and supporting documents, if any. You may also be required to grant the medical examiner access to your previous medical reports if required in relation to the medical examination.
- For female candidates who are menstruating, please postpone your medical examination to 1 week after the end of your menses. This is because menstruation will affect the results of the urine test.
- Please come in appropriate attire, as you will have to undergo a full physical examination.
- You are advised to wear a plain blouse or T-shirt which has:
- NO buttons
- NO embroidered or printed logos
- NO embroidered or printed designs
- NO pockets
- NO collar on the shirt
- The following Items are NOT allowed during x-ray examination:
- If your attire is found to be unsuitable for x-ray examination, you will be asked to purchase a disposable x-ray gown at a nominal fee.

4. |
Qualifying English Test (QET) (For undergraduate students only) |
| |
|
Students who fail to meet the English Language requirements will have to sit for the Qualifying English Test (QET). The purpose of the QET is to discern the level of English Language support appropriate for such students. Please click here for more details. |

5. |
Module Registration for the Special Term (only for returning NS men enrolling in the Special Term) |
| |
|
Returning NS men who wish to commence their studies in the Special Term (i.e. in May and June) must complete the Special Term Module Registration by the end of the designated period. Those who fail to do so will automatically be treated as commencing their candidature in Semester 1 of the academic year (i.e. in August).
Module registration for the Special Term will be held in March (Part 1) and June (Part 2) and should be completed online. Do note that you must complete the online Pre-registration before you can proceed with the Special Term Module Registration. You have to use your Student Number and PIN to access the online system, both of which could be obtained via the Pre-Registration System. The Special Term Module Registration results will be sent to your NUS student email account. |

6. |
Declaration to Start Course on Full Credit Transfer or on Clean Slate (for NUS undergraduate students transferring to another course of study) |
| |
|
NUS undergraduate students who are successful in their application to transfer to another course of study must declare if they wish to start the new course on a clean slate or with a full credit transfer. Starting the new course of study on a clean slate means there will be no waiver, credit transfer/exemption of modules. Please note that students transferring to Dentistry, Law or Medicine will have to start on a clean slate as the transfer of modular credits and CAP is not applicable to these courses. Those opting for a full credit transfer should note that modules accepted for credit transfer/exemption/waiver must not have been used towards earning a degree at NUS or another institution and the minimum residency requirement is applicable to the new programme of study. Faculty of Engineering students who are transferring to another course of study within the Faculty will have to opt for full credit transfer.

Estimated Time Needed for the Registration Exercise |

While every effort will be made to minimise waiting time, please be prepared to spend from 15 to 30 minutes to complete the registration formalities at the registration centre due to the large number of incoming students involved. However, this does not include the time needed if you wish to enquire about or purchase notebooks from the computer vendors stationed at the registration centre as part of the notebook ownership scheme and to participate in the matriculation fair organised by the NUS Students' Union and located at Multi-Purpose Sports Hall II. |

5. |
Participate in the Matriculation Fair |
| |
|
The Matriculation Fair is located at Multi-Purpose Sports Hall II and all freshmen are strongly encouraged to participate in it after completing the registration formalities in Multi-Purpose Sports Hall I. This fair, organised by the NUS Students' Union (NUSSU), is an avenue through which NUSSU and the various student clubs and societies reach out to freshmen, in order to build in them a sense of pride and belonging as part of the NUS family. Beside excelling academically, students can also fully participate in the host of cultural and social activities organised by these student bodies. |

Diagnostic English Test (for graduate students only) |
| |
|
Unless exemption has been granted, all international students are required to sit for the Diagnostic English Test (DET), which will determine if a candidate is required to read additional Graduate English courses. Please click here for more information about the Graduate English Courses offered.
The DET, conducted by the Centre for English Language Communication (CELC), requires a candidate to write an essay of 350 words based on a topic given. For the August 2007 intake, it will be held on 3 August 2007, from 10.00am to 12.00 pm. at Lecture Theatres 2, 5 and 6.
Students who are required to sit for the test should register online between 31 July 2007 to 2 August 2007 (by 5:00 pm). Those who are unable to do online registration should proceed to register personally at CELC, which is located at 10, Architecture Drive. |
