Please note that OSHE has revised its risk assessment form from the current twenty (20) pages to eight (8) pages.

The newer forms follow the same format, except greater emphasis would be placed on FSO/OSHE staff conducting on-site verification.
If you have submitted your Risk Assessment using the previous version, OSHE will still process it, you need not re-submit the risk assessment using the revised version.
