1. Please submit the following supporting documents after completing the online scholarship application form*:
   
   
1. A print-out of the “Submission of Documents for NUS Undergraduate Scholarships / Awards Application” page^, affixed with a recent passport sized photograph.
2. GCE ‘A’ Levels examination results, Polytechnic Diploma results or High School examination results.
3. One referee’s report (to be completed by your JC/Polytechnic/High School Principal or Tutor). 

Note: The referee’s report is confidential and should be returned to us in a sealed envelope with the flap bearing the signature of your referee.

4. JC/Polytechnic/High School CCA records.
5. Any other relevant award certificates.
6. A clear copy of your NRIC (front and back).

IMPORTANT: Singapore-Cambridge GCE ‘A’ levels candidates must submit an application for admission to the undergraduate courses by 1 April 2008. Polytechnic candidates and candidates with international high school qualifications must submit an application for admission to the undergraduate courses by 21 February 2008.
